Information about our program and enrollment forms can be obtained by calling the school office at 570-7822 ext. 0, or by emailing our enrollment coordinator Cindy at cintaylor@emerson.edisonschools.com. You may also download the enrollment form using the link below. The form is in PDF format and you will need Adobe Reader to open the file. If you do not have Adobe Reader you can download it using the link at the bottom of the page. You will need to print out the form and fax or send it to us. Our temporary fax number is 719/380-0176. Birth Certificate
Social Security Card
Immunization - shot records
